Scheme Manager - Part-time 25 hours per week

Salary £16,313 per annum (based on 25 hours)

Permanent opportunity for an experienced Scheme Manager to work 25 hours a week, providing care and support services across the scheme.

Responsibilities:
- Management of scheme staff
- Ensuring policies and procedures and guidelines are adhered to.
- Carry out one-to-one supervision meetings and annual appraisals
- Make daily contact with all service users
- Take referrals and carry out assessments
- Liaise with Social Services, Health professionals and other agencies as appropriate
- Ensure individual support plans are agreed and reviewed on a regular basis

Applicants must have previous experience within supported housing and knowledge of social care for older people.
Position is subject to an Enhanced CRB
